Knee Ligament Injuries

The knee is one of the most commonly damaged joints in sports, and damage to its stabilizing ligaments can be debilitating. Ligaments are the powerful tissues that connect bones to each other and provide stability.  

  • ACL (Anterior Cruciate Ligament) Tears: A common injury in sports requiring sudden stops and changes in direction like soccer and basketball. Patients often report a loud “pop” in the knee at the time of injury, followed by rapid, significant swelling and a feeling of instability or the knee “giving way”. While some partial tears can be managed with therapy, complete tears in active individuals often require surgical reconstruction to restore knee stability and allow a return to sport.  
  • PCL (Posterior Cruciate Ligament) Tears: The PCL is the strongest ligament in the knee and is injured less often than the ACL. It is typically torn by a powerful, direct blow to the side of a bent knee, such as a fall in football or a dashboard injury in a car accident. Many partial PCL tears have the potential to heal on their own with non-surgical treatment like bracing and physical therapy.  
  • MCL (Medial Collateral Ligament) Tears: The MCL runs along the inside of the knee and is most often injured by a direct blow to the outside of the knee, which forces the joint inward. Symptoms include pain and energy on the inner knee, swelling, and a feeling of instability. The vast majority of MCL injuries, even complete tears, can be treated successfully without surgery, using a combination of rest, bracing, and a structured rehabilitation program.

The menisci are two C-shaped pieces of tough cartilage that act as the knee's shock absorbers. A damaging twist or rotation of the knee, especially while bearing weight, can cause a tear. Symptoms include a popping sensation, pain, stiffness, swelling, and a feeling of the knee closing or catching. Treatment depends on the type and size of the tear. Some smaller tears can be handled conservatively, while larger tears that cause mechanical symptoms often require arthroscopic surgery to either repair the torn cartilage or trim away the damaged portion.

Common Overuse Injuries

  • Tendonitis: This is a rash of a tendon, typically caused by repetitive motion and overuse. It often presents as a dull ache that worsens with activity. Common forms in athletes include “jumper’s knee” (patellar tendonitis), “tennis elbow,” and Achilles tendonitis. Therapy focuses on the R.I.C.E. protocol (Rest, Ice, Compression, Elevation), anti-inflammatory medication, and physical therapy to correct biomechanical issues. For chronic cases, PRP injections can be very effective.  
  • Muscle Strains (Pulled Muscles): A muscle strain occurs when muscle fibers are overstretched or torn, usually due to fatigue, overuse, or an improper warm-up. Symptoms include a sudden onset of pain, soreness, swelling, and sometimes bruising. Initial treatment involves the R.I.C.E. protocol, followed by an incremental return to activity with gentle stretching and strengthening exercises.  
  • Stress Fractures: These are tiny breaks in a bone caused by repetitive force and overuse, common in runners and athletes who rapidly increase the intensity or volume of their training. The primary symptom is pain that worsens during activity and improves with rest. The most crucial treatment is rest from the high-impact activity to allow the bone to heal, which can take 6 to 8 weeks. During this time, cross-training with low-impact activities like swimming or cycling is encouraged.
